Type 2 diabetes mellitus and obesity have emerged as interconnected global epidemics, often termed “diabesity”. More than 90% of newly diagnosed T2DM patients are overweight or obese, highlighting the metabolic link between excess adiposity and glucose dysregulation. While lifestyle modification remains foundational, many patients require pharmacotherapy. Sodium-glucose co-transporter 2 inhibitors represent a novel class of antidiabetic agents that address both hyperglycemia and weight gain through an insulin-independent mechanism. By blocking renal glucose reabsorption, these drugs promote urinary glucose excretion, resulting in sustained caloric loss and 2-4% body weight reduction. Clinical trials have additionally demonstrated cardiovascular and renal protective effects, making SGLT2 inhibitors particularly beneficial for obese T2DM patients with high cardiometabolic risk. This review explores the pathophysiological basis of obesity-driven T2DM, the pharmacology of SGLT2 inhibitors, their clinical efficacy in weight management, comparative data with other agents, and the pharmacist’s role in patient education.{1}
