Abstract: In the traditional healthcare system, maintaining Medico and legal records manually has been a major challenge due to issues like misplacement of documents, lack of centralized access, time delays in retrieving patient data, and security concerns. Doctors often rely on physical prescriptions and scan reports, which can easily be lost or tampered with. Patients, on the other hand, struggle to keep track of their Medical history, especially during emergencies or legal proceedings. Furthermore, verifying the authenticity of Medico records in legal cases becomes complex without a proper digital trail. The Medico Legal Record Management App is a secure and streamlined system developed to manage digital Medico records with legal accountability. This application is designed with three main modules: Admin, Doctor, and Patient. The Admin is responsible for handling the approval and monitoring of the system. Doctors and Patients can register within the application, and their registration requests are forwarded to the Admin for verification. Upon approval, users receive an OTP for account activation. Only after successful activation can Doctors and Patients log in to the application, ensuring a secure entry point. Once logged in, Doctors can add prescriptions and upload scan reports directly to the app. Patients, upon logging in, can receive their prescriptions via email, ensuring easy and direct access to their Medico records. Additionally, patients receive email notifications when a scan report is added by the doctor. The system emphasizes transparency and accountability, as the Admin can track both patient and doctor login activities including date, time, and details of shared prescriptions. This app provides a centralized digital platform to manage Medico records, enhances the communication between doctor and patient, and supports legal traceability for all actions taken. It also minimizes paperwork, reduces human error, and strengthens patient data confidentiality.
