Cardiovascular diseases (CVDs) remain the leading cause of morbidity and mortality worldwide, driven by interconnected mechanisms such as oxidative stress, chronic inflammation, endothelial dysfunction, and metabolic disturbances. While pharmacological therapies form the backbone of CVD management, their long-term use is often limited by adverse effects and incomplete prevention of disease progression. In recent years, botanical interventions have gained considerable attention for their multifaceted cardioprotective effects and favourable safety profiles. This review explores the pathogenesis of CVDs and examines a range of medicinal plants, including Moringa oleifera, Curcuma longa, Camellia sinensis, Allium sativum, Berberis spp., Brassica vegetables, Panax ginseng, and Ginkgo biloba. The bioactive constituents of these botanicals, such as flavonoids, alkaloids, polyphenols, and terpenoids, exhibit antioxidant, anti-inflammatory, antihypertensive, lipid-lowering, vasodilatory, and antithrombotic activities. Mechanistic insights highlight their potential to modulate oxidative pathways, reduce pro-inflammatory cytokine expression, improve endothelial function, and regulate lipid metabolism. Although preliminary clinical studies support their cardiovascular benefits, challenges such as standardisation, herb–drug interactions, and limited large-scale trials hinder their integration into modern therapeutics. Future research should focus on rigorous clinical validation and standardised formulations to ensure safe and effective incorporation of botanicals into cardiovascular disease prevention and management strategies.
